Annual Well-Child & Sports Physicals

Purpose
Universal Health Certificate (UHC)

Note:
The Universal Health Certificate (UHC) is only valid for one year. Parents/Guardians are encouraged to schedule their student’s annual well-child visit every year on their birthday to receive the physical examination and any vaccines that they may be behind on.

DC requires all students to be up to date on vaccinations in every grade. Visit the link below to learn more about which vaccinations your student would need per grade.

Childhood Vaccination Guide by Age

No Shots, No School! is a DC policy that states that all students in grades Pre-K3, Kindergarten, 7th and 11th grade are at risk of being excluded from school and school-related activities if they are not up to date with the required routine childhood vaccinations for their current grade. A valid* Medical Action Plan (MAP) is required for certified school health staff to administer medicine/treatment to students while at school. See a few required forms below:
